Fire Incident Request Form

  1. Please use this form to request a Fire Incident Report. Please provide as much information regarding the incident as possible. The report can be emailed, mailed, or picked up in person at 1000 Lincoln Avenue. Once the report is ready, you will be notified via the phone number provided or by email (if you have selected to receive the report via email).

  2. Please note that the Fire incident number is not the same as the Police incident number or case number.

  3. If you are requesting a report related to a medical call, a subpoena or signed patient authorization is required to release any patient information. You may attached a digital copy here. Subpoenas must also be mailed but attaching a digital copy may expidate the release of the report.
