    This form should be completed when requesting to do a Ride-Along with the Woodland Police Department. This is solely a request for permission to accompany a member(s) of the Woodland Police Department during the active performance of their official duties as a police officer(s). Completing this application does not ensure the ability to do a ride-along. You will be contacted by the department with the outcome once the application has been reviewed and processed.

  67. Background Authorization*
    I understand that a criminal check and a warrant check will be conducted as part of the application process. I hereby authorize any law enforcement agency, agencies of the government of the United States of America and agencies of the State of California to release to the Woodland Police Department any and all information which said agencies or any of them have about me, for the limited purpose of aiding the Woodland Police Department in evaluating my eligibility for participation in the Ride-Along Program. This release extends to any and all information which said agencies or any of them may have about me, whether public, personal, or confidential. I understand that I will not receive and am not entitled to know the contents of confidential reports received from these agencies. I further understand that these reports are privileged. I hereby release, discharge, and agree to hold harmless the agencies, their agents and representatives and any person furnishing information from any and all liability of every nature and kind arising out of the furnishing and inspecting of such documents, records and other information, and this release shall be binding on my legal representatives, heirs and assigns.
  69. Age Requirement*
    I understand that in order to do a ride-along, the participant must be a resident of Woodland and sixteen (16) years of age or older. The signature of a parent or guardian is required for those participants under the age of eighteen (18) years old.
  71. Electronic Signature Agreement*
    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.

    After your Ride-Along Program Application is submitted, it will be reviewed. You will be contacted with the outcome of your application.
